3.  Are you a School of Computer Science (CTI) student? If not state the 
    School (LA&S, Education etc.).

4.  Are you an undergraduate or graduate student? Please state your major 
    (or concentration). If undecided state a likely major (or concentration). 

5.  Have you ever taken a statistics or probability course? If so, name the 
    course, the level and state how long ago? 

6.  Have you ever used SAS? If so how familiar are you with "Base SAS"?

7.  Will you be using SAS in the lab, will you be using SAS for Unix, or do 
    you need a SAS license for home?

8.  If you are a Distance Learning student please answer the following:

    a) Please supply a preferred postal address. SAS license materials and
       some course handouts will be mailed to the supplied address. If you
       plan to collect these materials at the CTI office state "CTI office".  

    b) Please indicate the campus where you expect to sit exams and quizzes.
       If you are a "Remote" student then state "Remote" and indicate the
       exam center (if known).
