Dear Applicant:
Thank you for your interest in the PLuS Program at DePaul University.
Please be aware that admission to the PLuS Program is separate from admission to DePaul University. You must be accepted by DePaul University to be eligible for PLuS services. Please review carefully the following forms, and complete the ones that are relevant to your case. Upon receipt of your completed forms, your eligibility for the program will be considered.
Along with any forms you will be sending to PLuS, you should include your most recent psychoeducational evaluation which has a diagnosis of a Learning Disability and/or Attention Deficit Disorder. This evaluation should not be more than five years old for LD, or more than three years old for AD/HD. This might be the most important document you send to PLuS; without this document PLuS cannot provide any services or accommodations to you.

To access the forms, click on the links underneath the description of each form. Microsoft Word will open the requested file. If asked if you want to open or save the file, choose to save it in your computer. After saving the file (make sure you remember where you saved it), open the file and you can either type your answers or print it and complete the form; all files are in Microsoft Word format.

4Permission to contact the Office of Admissions at DePaul University If you believe that PLuS could facilitate in the decision making process with the Office of Admissions, please sign and return this form. PLuS will contact the Office of Admissions on behalf of an incoming student on rare occasions, and only for reasons related to the LD and/or AD/HD diagnosis. Please note that the Office of Admissions will ultimately decide for your admission or not at DePaul University based on all the information you provided, and PLuS' letter of recommendation does not guarantee acceptance. Finally, in order for PLuS to advocate for you, you must meet with the PLuS director and discuss this matter with him.

4Permission to contact Professional In the event that the PLuS Program needs further information or clarifications regarding your strengths and weaknesses and/or your diagnosis, or if you want the PLuS Program to contact any professionals who worked or are working with you (LD specialists, psychologists, psychiatrists, etc.) please complete, sign and return this form to PLuS.

4Release of Information Letter, and Professional’s Report If you want any professionals who worked or are working with you (LD specialists, psychologists, psychiatrists, etc.) to contact and/or mail information to PLuS, please sign the release of information form and forward it to them along with the report form. Use one pair of these forms for each professional.

4Verification of Attention Deficit Disorder If you have been diagnosed with Attention Deficit Disorder, forward this form to the professional who made the diagnosis or who can verify the diagnosis.

4Academic Resource Center (ARC) Consent Statement This form outlines what you need to do if you decide to take the placement tests, which DePaul University requires upon admission, with extended time. It also gives permission to PLuS to contact ARC on your behalf, and directs you to the Office for Students with Disabilities (OSD) in the event you require accommodations due to a disability other than LD and/or AD/HD.
