| Grad LA&S Info. . . Quick update on the Grad LA&S website, The following forms are available on the Grad LA&S Website: http://condor.depaul.edu/~lasgrad/forms/gradforms.html Degree Conferral Application (this new form requests information for commencement ceremony participants as well). LA&S confers degrees at the end of each quarter. Please note: The deadline for degree conferrals is - Conferral Date: Deadline to submit application: The students no longer fill out two forms. It is now one form. The process works like this: Upon receipt of a student’s application, LA&S generates a letter that explains the process and progress of events that happen within the College and the Academic Resource Center and how LA&S determines if students have met all requirements. LA&S encourages students to check their Course History Report on Campus Connect and check with department for their requirements. The catalog is on-line (available through Campus Connect.) The degree conferral date is what will be listed on a student’s official transcripts and diploma. Degrees do not get posted on transcripts until at least 4 weeks after the degree conferral date. Diplomas take anywhere from 8-12 weeks after the degree conferral date. If students are requesting transcripts they need to check the box "when degree is conferred" otherwise the ARC will produce transcripts without degree posted. Final Projects Approval: This form is used in the beginning stage of a student's thesis/dissertation/writing project. Once a topic is approved by the chair of the committee, it needs to be signed and sent to the College Office to be filed in a student’s records. Students must register for appropriate thesis hours as well. Final Requirements: This form should be completed at the end of a student’s career after the thesis/dissertation/comprehensive. Students must use this form and send it to the College Office. If a student is doing a thesis, the student must bring this form to me when bringing in their final copies of the thesis/dissertation. Independent Study: There are still old forms floating in several departments. Please download this form and submit to College Office as needed. Readmission & Reclassification: If students have not been active for more than a year they must submit this form signed by their Program Director to be re-admitted into the program. This form must be submitted before the start of the quarter so that LA&S can activate the students record and the student then will be able to register, use their student ID, etc. Students who want to reclassify to another program must submit all materials to the program they are applying to as if they were applying to the program for the first time. This does not guarantee the student will automatically be accepted into the program, for the application is determined just like a normal application. Sample Title Page: for a student's thesis/dissertation…Please remind students to make sure that the date on this page coincides with their conferral date (Month/Year). If this information is not correct the student will be notified to re-do title page. Thesis Submission Form: Students must download and complete this form when turning in copies for their thesis/dissertation. We now have two options for this - unbound paper copies (as we currently do) and electronically (disk) PDF format only Student need to bring this form, the fee and their copies/disk to the College Office before their degree conferral date. The submission of their thesis/dissertation is a part of their graduation requirements and without this, their degree will not be conferred. Thesis/dissertation guidelines can be found at http://condor.depaul.edu/~lasgrad/forms/thesis_guidelines.pdf Transfer Credit Request: Students submit this form, their official transcripts, and a copy of the course description or syllabus to their Program Director for approval. The Program Director must note the equivalent DePaul course title and class number on this form and send it to the College Office. LA&S anticipates having more information on the web in February.
