    * Proposal defense (date)
    * Request for human subjects approval (date)
    * Past achievements
    * Future Goals/Expectations
    * Current Status
    * Comments/Remarks/Considerations

 2. Publications/Conference Presentations
    * Submissions
    * Poster presentations
    * Talks

 3. Working Relationship with Thesis Advisor:
    * Frequency of research meeting
    * Continuing interest in the present research project 
 

II. Records of Course Work:

 1. Course grades for the year

 2. Independent study
 

III. Records of Fellowship/Assistantship Work:

 1. Hours of work per week as T.A. (for whom)

 2. Hours of work per week as R.A. (for whom)

 3. Extra duties?

 * Both T.A./R.A. and Advisor information will be included in 
   an evaluation form, which must be submitted with the portfolio.
